Required Documents
Failure to submit all required documentation will result in a delay in the student’s start date.
Required documentation must be uploaded during the application process. If you are having difficulty uploading a document due to size restrictions, please save as a Reduced Size PDF (File>Save As Other>Reduced Size PDF). Please upload documents in either a PDF or JPEG format.
- Birth Certificate or student Passport(Obtain a new birth certificate)

- Physical must be completed within the past 12 months . You may submit a copy of the physical & immunization records provided by the student's physician or use thestate form.

If residence is a mobile home, please provide the following:
Proof of Home Ownership (23-24Berkheimer Real Estate Tax Bill, most recent Mortgage Statement or Deed)
A full copy of your current Lot Lease: Please include all pages from page 1 up through and including the signature page. Please ensure it is signed by both Landlord and Tenant(s).
The following areNOTacceptable forms of proof of residency:Utility bills, Certificate of occupancy, Title insurance policies, township or county tax bills, Annual mortgage interest statement, Local or federal tax statements
- IEP/GIEP/504 plan/Special Education documents (if applicable).

- Application for Multiple Occupancy Registrationform and proof of occupancy- required only if you are not on the lease or the home purchase/ownership form. This form must be notarized and you will be required to show four (4) forms of proof that this is your permanent address. (Spanish version)
- Application for Enrollment of Prospective Resident form and Agreement of Sale(must include all pages and be signed by all parties).Partial or unsigned documents will not be accepted. TheAgreement of Sale must include the settlement date; if it does not, please also provide a signed letter from the builder, stating your estimated or scheduled settlement date. (Spanish version)
- Authorization to Release Previous School Records form(Spanish version)

- Guardianship Affidavit- required if the student is residing with someone other than the parent. (Spanish version)
- Original Placement Letter- required if the student is placed with a family by a court system or foster agency

Age Requirements
- Kindergarten:students must have attained the age of five (5) years on or before September 1st of theyear of enrollment.
First Grade:students must have attained the age of six (6) years on or before September 1st of the year of enrollment.
Important Change in Definition of “Compulsory School Age”
Effective in2020-2021, all children from the age of six (6) through the age of eighteen (18) must comply with compulsory school attendance requirements. To meet these requirements, parents must ensure that their child between the ages of 6 and 18 is attending or participating in one of the following: a public elementary, middle, or high school; a public charter or cyber-charter school; a private licensed academic or private religious school; home tutoring by a certified teacher; or an approved program of home schooling. Children may be excused from compulsory attendance for documented medical or other compelling reasons as outlined in District attendance policy and procedures.
Kindergarten Registration
Kindergarten Age Requirement:students must have attained the age of five (5) years on or before September 1st of the year of enrollment.
AM/PM Kindergarten Preferenceisrequested in the application and is taken into consideration by the child's school.AM/PM placementinformation will be made available in July andteacher assignmentswill be announced mid-August.Bus informationwill also be announced mid-August.
**Registration for Kindergarten Enrichment through A Child's Place (ACP) will open in the Spring. You may register with ACP while Central Registration processes your application. AM/PM assignments are made by the schools and will be communicated in July; the schools will work with ACP to place students in the correct session.

Address Change for Current Students
Please communicate all address changes to your school. One of the following forms of Proof of Residency must be submitted to the school to process an address change:

The school will notify the Transportation Department and will provide you with the new bus schedule once finalized. If the student will be moving from one DASD school to another as a result of the address change, please contact the school they are leaving toinitiate thetransfer process. Once complete, please contact the new school to complete the transfer process.Note:Transportation changes take 8-10school days to finalize. Please be prepared to provide transportation until you receive notification from the Transportation Department.
Student Withdrawals
If you are moving out of the Downingtown Area School District or transferring your child to a non-DASD school, please contact your child's school to complete the withdrawal process.

Important Dates and Requirements
August 1st:
- Deadline for submitting the Home School Affidavit and the other required documentation for returning families. New families are free to submit documents at any point in the year.

- Physical report – due with 1st year of Homeschool and again in 6th and 11th grades

Special Education Requirements:
- If your child has ever been identified as requiring special educationservices, youmusthave the goals and objectives reviewed by either a teacher with a valid certificate from the Commonwealth to teach special education or a licensed clinical or certified school psychologist. This written notification of approval must be submitted with the notarized affidavit and goals and objectives no later than August 1st of the applicable school year.
JUNE 30th:
- Deadline for submitting current year evaluations
Note: Standardized Testing must be completed in Grades 3, 5 and 8. The evaluator report must confirm the testing has been completed and the results are satisfactory. It is not required to submit the results to the school district.
Pennsylvania Dept. of Education Mandatory Courses:
- Elementary school level (grades K-6), mandatory courses: English, to include spelling, reading and writing; arithmetic; science; geography; history of the United States and Pennsylvania; civics; safety education, including regular and continuous instruction in the dangers and prevention of fires; health and physiology; physical education; music; and art.
- Secondary school level (grades 7-12), mandatory courses: English, to include language, literature, speech and composition; science; geography; social studies, to include civics, world history, history of the United States and Pennsylvania; mathematics, to include general mathematics, algebra and geometry; art; music; physical education; health; and safety education, including regular and continuous instruction in the dangers and prevention of fires. Such courses of study may include, at the discretion of the supervisor of the home education program, economics; biology; chemistry; foreign languages; trigonometry; or other age- appropriate courses as contained in Chapter 5 (Curriculum Requirements) of the State Board of Education

Important Notes:
- Effective with the 2020-21 school year, a child must comply with compulsory attendance from ages six to 18, or until graduation. Homeschooling must be documented and evaluated to account for all schooling during that time.
- Written notification must be received when you discontinue a Homeschool program to enroll in another school or move out of the Downingtown Area School District.

Special Education Evaluation
Please see below for the first step in requesting an evaluation for Special Education, Section 504 or Gifted Education services:
For students who are currently enrolled in a DASD school: Please contact your student’s school to submit your request for an evaluation.
For students who reside within the DASD but are currently attending a private, charter or non-DASD cyber school: Please email a written request to the Director of Pupil Services, Dr. Ann Berrios at ABerrios@dasd.org Please be sure to include the reason for your request in theemail. Once your request has been approved by Dr. Berrios, she will provide you with instructions for the next step.
